Multiple Primaries--Brain and CNS: How many primaries should be recorded in a patient with von Hippel Lindau disease that has a hemangioblastoma of the cerebellum in 2003 and a hemangioblastoma of the brainstem in 2007?

Discussion:

Answer:

A tumor of the cerebellum (C716) and a tumor of the brainstem (C717) are multiple primaries because the topography codes are different at the fourth character of site.

In the previous editions of the SEER manual and benign brain MP/H rules, benign brain tumors represented different primaries if the first two numeric digits of ICD-O-3 topography codes differed. Per NPCR Training Materials, benign brain tumors are different primaries if all four digits of the site code differ. Rule M4 of the MP/H rules has since been corrected to read: Tumors with ICD-O-3 topography codes that are different at the second (CXxx), third (CxXx) and/or fourth (CxxX) characters are multiple primaries.
